North Respite Survey

Aberdeenshire Health and Social Care Partnership are currently conducting a review of Respite provision in the North of Aberdeenshire. As part of this review, we would be grateful if you could complete this short questionnaire on whether you have accessed respite, the type of respite you have accessed and your experience of using respite. 

Within this short questionnaire we will refer to 'Building based' and 'Creative Option', a definition of what we mean by each is as follows:

Building based - A specialist on-site building from which adults with learning disabilities can receive respite care. An example of this would be Willowbank in Peterhead. 

Creative Option - Respite which can be funded through self-directed support budgets Option 1 or 2. This could be a short stay in a Hotel, an activity break or use of a PA.
