Service User Survey

You should complete this survey if you have been involved in an Adult Protection visit or meeting and have been asked for your feedback.  If you have not been asked for feedback you can still give you views using my story tool.

This survey will ask you twelve questions related to your experience of adult protection.    The survey will take approximately five minutes to complete. Answers given will not be linked to you as an individual and you will not be asked to give your name.
