Community Survey - South Aberdeenshire Dementia Services

Do you have experience of living with dementia or caring for someone that does or do you have a view about how dementia services should be shaped for the future? If yes, please help us to design the future of dementia assessment services in South Aberdeenshire by completing this survey. 

This redesign is aiming to evaluate the needs of people living with dementia and their carers to determine a new way of providing our services. 

You will be asked to indicate whether you are a person living with dementia, a carer / family member of someone with dementia, or if you are someone with an interest in this area but no direct experience. Thank you for your participation in the survey.
