Dementia Diagnosis and Post Diagnostic Support Survey

To help us understand people's experiences of receiving a diagnosis of dementia and the support that followed, we have developed a short survey which will really help us to shape how we deliver services in the future.  

People living in Aberdeenshire receiving a diagnosis of dementia, should be offered one year of extra support to help them and their family, this is known as ‘post diagnostic support’.

The post diagnostic support service is meant to deliver support in the year following a dementia diagnosis to individuals and their families to help them understand the diagnosis, access the right support, and make empowered decisions to prepare for the future.

The survey will take around 15 minutes to complete and closes on 02 July 2023. Please find the survey below.
